IAX: Fix hangup SEGFAULTing the iax sound subsys + Add samplerate conversion...

IAX: Fix hangup SEGFAULTing the iax sound subsys + Add samplerate conversion to Mic output (still non-working)

Adding samplerate conversion and sound output fixed the problems of the
Jitter Buffer. It seems it updates the timestamps and causes less empty
packets crashing the system.

* Removed some debugging output.
* Cleanups
