Add a coding standard and apply it to the whole code. Also feature keyboard shortcut support, cleanup and minor modifications

Backport alternate volume control from a branch. We now have two volume control applet. The old one may be dissimissed if nobody have something against that move. Always on volume control have better accecibility.

Do not send DTMF when dialing

ManagerImpl::sendDtmf was returning without sending DTMF on AccountNULL

This callback is used when receiving INVITE request without SDP.

It makes SDP answer to be sent in ACK even if offer have been already
sent with INVITE request

This reverts commit 75a52d4c.

Branch ID must be the same, PJSIP send non-2xx
ACK automatically if not handled
