99 % of these are just whitespace changes. Some unnecessary includes
removed as well

sflphone-common -> daemon
sflphone-client-gnome -> gnome
sflphone-client-kde -> kde
sflphone-plugins -> plugins

remove a bunch of unused functions
remove a bunch of pointer indirection
remove a bunch of memleaks

video codecs are identified by their name rather than by their payload,
because for most (all?) codecs the payload is dynamic.

remove unused codecs bandwidth information

Removed unused methods, macros

This was only failing on the test machine because the tests were running
againt an outdated version of our plugins.
