[#1787] _audiortp->start() moved away from offhold(), SIPCallAnswered()

and handle_reinvite() to call_on_media_update. fetch_remote_ip_from_sdp
(renamed set_remote_ip_from_sdp) was misusing pj_str_t (which does not
have a null character) causing bad init in some cases.
