This avoids redundant "if (x == NULL)" style checks and puts the onus on
the caller to use valid parameters.

This reverts commit a4256fcb, reversing
changes made to aa2dd717.

History saving still has to be updated.

99 % of these are just whitespace changes. Some unnecessary includes
removed as well
