Improved media settings
- media device selections should be saved
- in the case of an incorrect device index, the selection should fallback to an appropriate default device
- the device list should respond to change in available devices
- input/output gain values should be exposed for clients
- threshold volume should be exposed for clients
- push to talk should be exposed for clients
